  • Supporting medical certificate from treating doctor; Attested copies of Indoor case papers.
  • Attested copy of First Information Report(FIR)/Medico Legal Certificate(MLC)
  • Investigation reports supporting the diagnosis
  • Itemized breakup of costs
  • Attested copy of Hospital registration certificate.
  • Settlement letter from other insurers

For Travel Claims:

List of admissible documents

  • All claim documents except original paid receipts for hospitalization.
